As a Registered Nurse you are passionate about bringing peace of mind to your client and their families. You are responsible for initiating Happier Aging experiences for Clients and Caregivers, you are responsible for the day to day operations related to driving business results through Client and Caregiver experience. You will work closely with your Care Designer to create a premium experience and deliver on client care goals.
Duties and Responsibilities
Creates Happier Aging experiences and client focused care in home and community settings
Delivers on our Brand Promise by owning the client’s experience, ensuring that you do whatever it takes to bring peace of mind to our clients and their families
Delivers comprehensive nursing care and assessments to clients according to your scope of practice outlined by CNO
Engages and reports in open communication with your Care Designer when opportunities to elevate your client’s experience and care are identified
Responds to Care Services Center (CSC) team regarding scheduling inquiries and updates within the agreed upon timeline
Function within the limits of your own experience and knowledge, while practicing safely and competently as outlined by CNO
Is a leader in the community
Able to use applications via phone and email to ensure proper communication and documentation are completed for your client’s care
Client Care Delivery:
Attends First Visits with Caregivers to ensure Client needs and care plans are executed as per the Service Agreement; sets and role models expectations
Conducts supervisory visits to ensure Caregiver fit and quality of care; delivering a premium client experience impacting Net Promoter Scores of 9 or higher
Reviews appropriate and individualized care plans and updates as required
Monitor risk issues and analyzes trends, adapts care as needed
Reviews care, implementing clinical solutions, and evaluates client outcomes based on client goals
Leadership:
Reports to the Care Designer
Delegates care and provides training and support when needed (ie. caregiver needing to use a ceiling lift, change a catheter bag)
Drives results through leading Caregiving team by setting clear goals, coaching in the moment and ensuring training needs are met
First and Supervisory visits to evaluate the Caregiver performance and job satisfaction, impacting the employee experience resulting in a Net Promoter Score of 9 or 10
Scheduling Collaboration:
Delivers exceptional Client service by achieving scheduling goals and responding to urgent inquiries
Responsible for ensuring Client coverage is secured, particularly due to urgent care, Caregiver cancelations and limited availability; assists with Client visits and scheduling as needed; able to cover shifts as required.
Administration:
Reviews and updates Client and staff records quarterly to ensure 100% accuracy and completeness
Reviews documentation and follow up with any care related concerns
Follows up with care plans and updates as needed

Qualifications:
Licensed Practical Nurse Certificate or Registered Nurse
CPR and First Aid Certification
Valid driver’s license
Healthcare background
Two positive professional references
Ability to travel independently to client visits
CPR and First Aid Certification
Intentional organization, planning, time management and multi-tasking skills
Exceptional written and verbal communication skills
We provide care 24/7, so visits may be scheduled early morning, afternoon, evening, or overnight to meet our clients’ needs.
Current practicing registration as a Registered Nurse with CNO license
Minimum 1 year RN experience, home health care experience is considered an asset
