As a Care Designer, you are responsible for the day-to-day operations related to driving business results through client experience and caregiver management. You will drive growth through the following main duties and responsibilities:
Duties
*Develop and execute new sales & marketing ideas to drive revenue growth
*Drive business interest by engaging in relationships with community leaders and professional organizations to establish long-term relationships that will translate into word-of-mouth and professional referrals.
*Responsible for planning and executing the monthly sales and marketing plan
*Conduct Caring Consults with Clients by assessing clients’ needs concerning their physical, psychological, and social aspects.
*Create a care plan for clients while collaborating with client/family/caregivers and other health care professionals based on the client’s needs and assessment.
*Provide nursing care to clients based on the care plan and delegate the tasks to other nursing staff and PSWs
*Evaluate the care plan and adjust accordingly in consultation with the client/family/caregivers/other healthcare professionals.
*Act as a patient advocate by protecting and promoting patient rights
*Promote patient safety and assist them in living comfortably in their home environment.
*Review client experience through customer feedback and make changes accordingly.
