The Licensed Practical Nurse / Registered Practical Nurse is responsible for providing skilled nursing care that supports safety, comfort, and quality of life for clients aging at home. Working in collaboration with a Registered Nurse and the broader care team, the LPN/RPN helps translate care plans into consistent, meaningful care experiences that promote Happier Aging.
Through sound clinical judgment, clear communication, and trusted relationships with clients and families, the LPN/RPN plays an essential role in delivering care that is both technically excellent and deeply human.
Essential Duties and Responsibilities
The Licensed Practical Nurse / Registered Practical Nurse delivers care with professionalism, accountability, and Bold Kindness, including but not limited to:

Provides direct client care within the scope of nursing practice and applicable legislative acts, using sound judgment and critical thinking.

Implements and supports plans of care and prescribed treatments in alignment with clinical direction and client goals.

Completes accurate, timely, and detailed documentation consistent with the nursing process, regulatory requirements, and organizational standards.

Conducts ongoing client assessments and monitors changes that may impact health, safety, or quality of life.

Participates in health education, therapeutic counseling, teaching, and emotional support for clients and families, recognizing their role in the Happier Aging journey.

Observes clients condition and communicates relevant findings to the Registered Nurse or appropriate clinical lead in a timely manner.

Assists with client discharge, transitions, or transfers to support continuity of care.

Assesses learning needs and provides education to clients and caregivers related to diagnoses, treatments, and care plans.

Prepares, administers, and documents medications as authorized within scope, including oral, topical, subcutaneous, and intramuscular medications.

Monitors, reports, and documents adverse reactions to medications or treatments promptly and accurately.

Recognizes the need for and initiates emergency measures quickly and appropriately, in accordance with training and policy.

Participates in the coordination of home health services, communicating identified needs for other disciplines (such as PT, OT, ST, or dietetics) to the Care Designer, Director of Nursing, or Registered Nurse.

Performs all skills, treatments, and procedures competently in accordance with provincial scope of practice, agency policies, and professional standards.

Follows principles of infection prevention, control, and universal precautions at all times.

Participates in performance improvement activities, contributing to a culture of learning and continuous quality improvement.

Uses equipment and supplies safely and appropriately.

Upholds client rights at all times, including the right to privacy, choice, and confidentiality.
Qualifications

Graduate of an accredited Licensed Practical Nurse or Registered Practical Nurse program from a recognized community college.

Current Certificate of Competence and registration in good standing with the applicable provincial College of Nurses.

Minimum of six months to one years experience.

trach care and gtube feeding experience preferred.

Current CPR certification.

Strong clinical assessment and technical nursing skills.

High level of interpersonal and communication skills.

Demonstrated ability to deliver excellent client service and interact respectfully with clients, families, team members, and community partners.

Valid driver’s license and access to an insured vehicle in accordance with organizational requirements.

Negative TB test or chest x-ray, as required.
Environment, Personal Protective Equipment, or Physical Demands
Environment:

Contact with clients under a wide variety of circumstances, primarily within the client’s home and occasionally in institutional environments such as hospitals.

May be required to respond to emergency situations.

Travel is required.

Position includes potential exposure to blood and other potentially infectious materials (OPIM).
Personal Protective Equipment:
- May include gloves, masks, and eye or face protection as required to safely perform care and follow infection prevention and control practices.
Physical Requirements:

Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull as required by client care needs.
