Responsibilities and Duties:
Responsibilities of the Certified Nursing Assistant include, are to, but are not limited to the following:
- Provide meal preparation, light housekeeping, and/or provide companionship and personal care
- Execute on the client’s care plan as outlined by the Care Designer
- Meet the safety needs of the client and use equipment safely and properly
- Function within the limits of your own experience and knowledge, while practicing safely and competently within the job description
- Complete records and documentation accurately and thoroughly
- Service the Scottsdale and Phoenix area
- Looking for all visit times, weekdays and weekends
- Provide client care as directed by the Care Designer or Nurse.

Provide personal care:
- Bathing
- Grooming, including oral care
- Assist the client with dressing and undressing
- Assist the client with toileting activities, including the use of the bedpan and urinal
- Provide catheter care including changing the urinary catheter bag
- Empty ostomy bags, or changing bags that do not adhere to the skin
- Assist in feeding the client, if necessary
- Feed clients who are unable to feed themselves
- Take and record vital signs (temperature, pulse, respirations and blood pressure)
- Measure and record height and weight, if necessary.
- Support good skin care to prevent skin breakdown (i.e. turn and reposition client, actively encourage movement)
- Assist client with active and passive range of motions as defined by the plan of care
- Provide client with help walking and transferring; assist with the use of devices for aid to daily living such as a wheelchair or walker;

Observe client’s physical, mental and emotional conditions
- Report any change to the Care Designer
- Document observed changes
- Measure and document intake and output, if necessary
- Assist client in the self-administration of medication
- Verbally remind clients to take their medications
- Perform light housekeeping; maintains a clean, safe and healthy environment, which may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the client’s or client’s dishes or laundry, and such tasks to maintain cleanliness and safety for the client;
- Perform meal preparation and clean up
- Change bed linens and make up the client’s bed
- Maintain an orderly client environment.
- Follow principles of infection control and Universal Precautions.
- Identifies and participates in performance improvement activities.
- Provide transport to doctor appointments, outside activities and shopping.
- Participates in client teaching according to the client’s plan of care.
- Assist in providing a safe environment and promote quality client care through adherence to established policies, procedures and standards.

Ensure client’s rights are adhered to. These include but are not limited to:
- the right to privacy and dignity while care is being provided
- the right to accept or refuse care
- the right to confidentiality of their information
- Respond rapidly and appropriately in an emergency situation
- Use equipment properly and in a safe manner.
- Perform other tasks as determined and instructed by the licensed professional and as permissible under Arizona law.
Requirements:
- 1 year previous caregiving experience.
- Valid Level 1 fingerprint clearance.
- Must have reliable source of transportation to and from client homes in the West valley, South Phoenix,Phoenix and Scottsdale Arizona area.
- Must be able to community verbally and in writing in English, other languages are an asset.
- Proof of CNA/HHA Health Care Certification (Registry #)
- Registered with the State Board of Nursing in Arizona
- Current TB Screen and Criminal Record Check.
- Must be able to legally work in Arizona .
- Certified in CPR and First Aid.
- Valid driver’s license is required.
- Treats clients, staff and the public with courtesy, respect and presents a positive public image.
- Works as a team member
- Ensures confidentiality and security of the client’s medical information.
JOB LIMITATIONS:
The Home Health Aide will not function in any manner viewed as the practice of nursing according to the State’s Nurse Practice Act. Specifically, the Home Health Aide will not administer medications, take physician’s orders or perform procedures requiring the training, knowledge and skill of a licensed nurse.
ENVIRONMENT, PERSONAL PROTECTIVE EQUIPMENT OR PHYSICAL DEMANDS:
Working Conditions: Contact with clients under a wide variety of circumstances. Care provided in a client’s living environment with varying situations or in an institution such as a hospital. May be required to respond in an emergency situation. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.
Personal Protective Equipment: May include gloves, mask, eye protection, and disposable outer covering.
Physical Requirements: Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 50 pound
