Responsibilities and Duties:

Responsibilities of the PT/OT include, are to, but are not limited to the following:

  • Conduct comprehensive physical and/or occupational therapy assessments
  • Develop and implement individualized treatment plans based on client goals
  • Provide therapeutic exercises, functional training, and rehabilitation techniques
  • Assist clients in improving mobility, strength, coordination, and daily living skills
  • Train clients in the use of adaptive equipment and assistive devices
  • Educate clients and caregivers on therapy plans, exercises, and safety practices
  • Monitor and document client progress and modify treatment plans as needed
  • Collaborate with physicians, nurses, caregivers, and other healthcare professionals
  • Maintain accurate and timely clinical documentation in compliance with regulations
  • Promote client independence and safety in home or care settings
  • Participate in care planning meetings and case conferences
  • Participates in client teaching according to the client’s plan of care.
  • Assist in providing a safe environment and promote quality client care through adherence to established policies, procedures and standards.
  • Ensure client’s rights are adhered to. These include but are not limited to:
    • the right to privacy and dignity while care is being provided
    • the right to accept or refuse care
    • the right to confidentiality of their information
  • Respond rapidly and appropriately in an emergency situation
  • Use equipment properly and in a safe manner.
  • Perform other tasks as determined and instructed by the licensed professional and as permissible under Arizona law.

Requirements:

  • Degree in Physical Therapy or Occupational Therapy from an accredited program
  • Current state licensure as a Physical Therapist and/or Occupational Therapist
  • Previous clinical or home care experience preferred
  • Strong understanding of rehabilitation principles and practices
  • CPR/First Aid certification (as required)
  • Holds a valid level one Fingerprint card
  • Ability to travel daily including weekends.
  • Understands regulatory compliance.
  • Must be able to legally work in Arizona .
  • Valid driver’s license is required.
  • Treats clients, staff and the public with courtesy, respect and presents a positive public image.
  • Works as a team member
  • Ensures confidentiality and security of the client’s medical information.


ENVIRONMENT, PERSONAL PROTECTIVE EQUIPMENT OR PHYSICAL DEMANDS:

Environment: Contact with clients under a wide variety of circumstances. Care provided in a client’s living environment with varying situations or in an institution such as a hospital. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.


Personal Protective Equipment: May include gloves, mask, eye/face protection, and disposable outer covering or lab coat.

Physical Requirements: Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the ability of the client. Must be able to lift up to 50 pounds. Visual and hearing acuity required
